Perimenopause, hot flushes, sleep and the midlife years
Forty-four, sleeping badly, furious by Wednesday, and told your bloods are normal. Perimenopause can run for years before a single test shows anything, and being told nothing is wrong is not the same as feeling well. Our menopause support Auckland team investigates properly before recommending anything.
The midlife years
Perimenopause is the run-up, and it can last from a couple of years to ten. Cycles shorten and then lengthen, sleep goes first, mood becomes unrecognisable, joints ache for no reason, and the brain fog is bad enough that plenty of women quietly wonder about something worse.
What makes it hard to get help with is that hormone levels swing wildly during perimenopause, so a blood test taken on the wrong day can look entirely normal while you feel nothing of the sort. Symptoms and timing are more informative than a single measurement, which is why we spend the time on them.
We are not prescribers. Hormone replacement therapy is a decision for your GP or a menopause specialist, and it is a good option for a lot of women. What we add sits alongside it or alongside choosing not to take it: acupuncture, which has been studied for hot flushes and sleep with mixed results, plus investigation of thyroid, iron and nutrient status, and the nutrition side of the transition.

What we help with
Across the whole span, not just the menopause end of it.
Irregular cycles, broken sleep, mood swings, brain fog, joint aches and a shorter fuse. Often years before periods actually stop, and often dismissed.
The most common reason people ask us about acupuncture in midlife. Research results are mixed, and HRT is the most effective treatment for most women, so talk to your GP first.
Waking at three, drenched or simply awake. We look at what is waking you and what is worth trying. More on stress and sleep.
Irregular cycles, insulin resistance and the symptoms that come with it. We work on nutrition and the insulin side, alongside your GP’s care.
We cannot treat the tissue. We can work on the muscle tension around the pelvis and lower back that often comes with it.
Common in perimenopause and worth raising with your GP. We check iron levels, which heavy bleeding can quietly run down.
Being straight with you
We do not prescribe and we do not replace HRT. Menopausal hormone therapy is well evidenced, and for many women it is the single most effective thing available — if you want it, your GP or a menopause specialist is the right person, and we will encourage you to have that conversation.
Hormone testing in perimenopause also has real limits. Levels fluctuate enormously from day to day, so a single result rarely settles anything, and no test can tell you when your last period will be. Where testing genuinely helps is in checking the things that mimic or worsen menopausal symptoms — thyroid function, iron, vitamin D and blood sugar. That is where we start.

The testing
Selected to answer a question. If a test will not change what we do, we do not run it.
Thyroid problems become much more common in midlife and produce fatigue, weight change, mood shifts and irregular cycles that look exactly like perimenopause.
Heavy perimenopausal bleeding depletes iron quietly. Vitamin D, B12 and magnesium also affect energy, mood and sleep in this decade.
Useful in specific situations — suspected early menopause, PCOS, or an unclear picture — and interpreted with the date of your cycle, not in isolation.
Stress and menopause amplify each other. This test shows how your cortisol rises and falls across the day.
Central to PCOS, and relevant to everyone in midlife as insulin sensitivity shifts. Often the most actionable thing we find.
Urine-based testing that shows how hormones are being processed rather than only how much is present. Used selectively, not routinely.
Who is involved
Most women use acupuncture plus one other. Nobody needs all four.
Calm, hands-on sessions for people who want a non-drug option alongside their GP’s care. Also used for period pain and pelvic tension.
Investigating what else is contributing — thyroid, iron, vitamin D, blood sugar, cortisol — so that a treatable driver is not simply filed under menopause and left.
Protein, fibre, calcium and steady blood sugar do more for midlife energy, bone and body composition than any supplement. Central to PCOS and to the change in metabolism after menopause.
For the joint aches, stiff hips and back pain that arrive in this decade and are commonly written off as simply getting older. Massage is relaxing in its own right.
Your first visit
Unhurried, thorough, and with an honest view of what is worth spending money on.
Cycles, sleep, mood, temperature, energy, joints, libido and brain fog, mapped against when each started. The pattern usually tells us more than a blood test would.
Thyroid, iron, vitamin D and blood sugar checked before anything is attributed to hormones, because all of them mimic perimenopause.
Acupuncture, hands-on treatment or both on the day, with a plan for how often and for how long before we reassess.
If HRT looks like the right option for you, we will say so and help you go into that appointment with a clear symptom history rather than a vague one.
Between appointments
Strength training is the one with the most evidence behind it in this decade, for bone density, body composition, mood and sleep. Twice a week, lifting something genuinely heavy for you, beats an hour of walking for these particular outcomes — though the walking is worth doing too.
Protein at every meal and enough fibre, with alcohol looked at honestly. Alcohol reliably worsens hot flushes and night waking, and it is often the easiest thing to test for yourself over a fortnight.
Keep a symptom record, especially of hot flushes and night waking, before and during treatment. It makes it obvious whether something is working, and it makes any conversation with your GP far more productive.
